Registration Procedure
- COMPLETE this registration form online
- SUBMIT this form electronically (or via fax to 352-273-8070)
- PRINT a copy for your records
- MAIL PAYMENT to ICBR (see address below)
- CONFIRMATION: After you finish the online registration, an automatic email confirmation will be sent to your email account. Within three business days of receipt of your online registration, you will receive the registration processing result via email or fax . If you do not receive it, please contact us.**
Payment Procedure
- Please make check payable to the UF RESEARCH FOUNDATION (FEID #59-2729133).
- To pay the registration fee for a University of Florida participant, prepare an Unencumbered Voucher via PeopleSoft (PS) (instructions available at http://wla.erp.ufl.edu/media/guides/UnencumberedVouchers.pdf).
- On the first PS screen (Voucher/Add a New Value) enter the following in the respective boxes:
- Vendor ID: 0000002522 (UF Research Foundation)
- Address Sequence Number: 2 (UF Research Foundation-ICBR)
- Invoice Number: Enter the Number generated on the Participant's ICBR Registration Form.
- Invoice Date: Enter the Date generated on the Participant's ICBR Registration Form.
- On the second PS screen under Invoice Lines, enter:
- ITEM: 80141902 (Meetings Events Registration)
- On the same PS screen under Distribution Line-Chartfields, enter:
- Department/Fund/Program information. The PS Account (Code) is 771100.
- Scroll to the right & enter the Participant's UFID (so we can match the payment with the participant on the Registration Form).
- On the first PS screen (Voucher/Add a New Value) enter the following in the respective boxes:
- If paying through an outside agency with a Purchase Order, please submit the PO to the address below and we will invoice the agency.
- Cash is accepted if paying in person.
- Credit cards are not accepted.

Fax: 352-273-8070 - RECEIPTS will be distributed the first day of the workshop, unless otherwise requested.
- Workshop registration fee must be paid by the first day of the workshop or your participation cannot be guaranteed.
- Workshop registration includes breaks, handouts, and a custom-designed laboratory manual. Additional copies of lab manual can be provided for a $25.00/unit fee.
- You may withdraw your registration and qualify for a refund minus a $100 processing fee, provided a written withdrawal request is received no later than two weeks before the event.
- No refunds will be provided after two weeks prior to the start of the course/workshop.
- Substitutions are permitted.
**The ICBR reserves the right to cancel any workshop/course if a minimums registration is not achieved three weeks prior to the event. In this case, registered participants will be notified immediately following cancellation and receive a full refund.
